Application form for mdx original account

This application form should be used for those who do not have an Gakunin account that is authenticated via Gakunin (https://www.gakunin.jp/en).  We call an “mdx local account” for an account that is authenticated via our own mdx’s two-factor authentication system rather than using Gakunin.
If there is no website which lists you in your organization’s official web page, we will ask you to submit a photocopy of identification document.(e.g. your organization card, driver’s license). Details will be announced individually.
